After participating in this course, you will be able to:

  • Understand the various methodologies used in problem solving & failure analysis & the ability to conduct & co-ordinate a multi-discipline team to investigate the problem & present mitigating actions
  • Learn techniques required to investigate failures
  • Gain insight into how a system fails & provide solution to prevent reoccurring

The participant will have an overview of various techniques & methodology used to perform Root Cause or failure analysis. This course will focus on technical & engineering failures but the methodology discussed is not limited to technical subjects; it has a wide range of applications, from business, medical, human performance, to non-technical issues. The final goal of any failure analysis is to identify the root cause of the failure under investigation & present effective mitigating actions to prevent the failure from reoccurring again.

The basic technique will be presented & case studies will be discussed. Participants will be involved in a hands-on workshop session to demonstrate their skills in problem solving. By the end of the seminar, the participant will be able to use this technique to lead a multi-discipline, cross-functional team to execute a successfully failure analysis. The course material is targeted to individuals with no or limited background on the subject. It is also presented as a refresher course for people who have prior experience in failure analysis.

  • The need for failure analysis
  • Why equipment/system failure
  • The life of a failed equipment/system
  • Various application
  • Examples of significant failures in history

Initial Stage

  • Problem identification
  • The terms of reference
  • Team approach and identify resources
  • Setting the ground rules

The Use of Various Techniques

  • Investigation Matrix
  • Information gathering and categorization (probabilistic verses deterministic analysis)
  • Potential failure modes/ Failure Mode analysis
  • Event and Causal factor/Fishbone chart

Analytical Phase

  • Key probable failure modes and review similar past failures
  • Define failure Scenario and recreation of the failure
  • Identify the root cause and other contributing factors
  • Reality check

Providing the Solution

  • Provide the path forward/recommend mitigating actions
  • Risk Analysis/metrics to be trended
  • Cost and benefit analysis/sensitivity analysis
  • Interim and Final report

Case Study 1

  • Using a simple case study to demonstrate the use of various techniques

Case Study II

  • Using a more complicated case study to illustrate the approach to root cause identification and multiple contributing factors in a failure analysis

Case Study III

  • Demonstrate a multidiscipline team approach to problem solving

Discussion of Various Engineering Failures and Failure Mechanisms

Workshop Session and Final Discussion

Questions and Answers and Feedback to Participants on Achievement of Learning Outcomes

Gee is a professional engineer working as an independent consultant in power utilities, petro-chemical, and mining and energy projects.

He has experience in the areas of piping design, stress analysis, system flow, plant layout, failure analysis and project management. He has provided expertise as a consultant in the field of mechanical & piping systems and he has been active as an instructor in related courses for over 25 years.
