Description
Root Causes Analysis (RCA) enables engineers to address immediate causes of failure but, more importantly, to investigate the underlying root cause. With modern methodologies for RCA, it is possible to identify and eliminate the root cause of a problem by employing suitable mitigating actions.
This course will present modern techniques used to perform RCA. The focus will be on various engineering failures. However, the methodology will also be extrapolated to a more comprehensive range of applications, including business, medical, and other non-technical fields. Several workshops with real-life case studies will be provided, where participants will gain hands-on experience in conducting the RCA analysis. Problems will be solved through teamwork, and the results will be analyzed and discussed.
This course is designed for technical and non-technical professionals with limited or no experience in the subject area and can be taken as a refresher course.
Who Should Attend
Professionals who are responsible for identifying potential failures and their successful solving.
Maintenance engineers and technicians, operators and technologists. Other technical personnel who are directly or indirectly involved in maintenance and repair activities. Non-technical personnel who want to learn about investigative RCA skills are required to troubleshoot any potential problem encountered.

- Importance of RCA analysis

- Common failure modes and causes: technical, human, etc.
- Post-accident investigation: gathering data and evidence
- Methodologies in RCA: Fault Tree method, Fishbone Ishikawa method
- Methods on the shop floor: 5 Whys, Event and Causal factor, Pareto method
- Workshop: Case studies and applications of RCA
Day-2
- Hazard identification and importance of diagnostics
- Risk assessment methodology: risk matrix
- Efficient risk mitigation techniques: ALARP
- Failure mode and effect analysis (FMEA)
- RCA used in troubleshooting industrial problems
- Reliability engineering and failures: Cost-benefit analysis
- Workshop: Case studies and applications
- Summary and Conclusions
FORMATEUR

Dr. Mike Ivanovic is the President of UBAMIP Consulting and a former Adjunct Associate Professor at the University of Toronto. With over 30 years of experience in various branches of mechanical engineering, he has established himself as a leader in the field. His career includes significant contributions while working in the consulting group of ANSYS Canada Ltd. He currently heads UBAMIP Consulting, where he specializes in the analysis and problem-solving of equipment operations within the process and petrochemical industries, as well as thermal and HVAC engineering.
Dr. Ivanovic's expertise encompasses safety aspects in the operation and maintenance of rotating machinery, pressure vessels, and piping systems across diverse industrial applications. His consulting projects primarily focus on risk assessment of potential equipment failures and the development of accident prevention methods, informed by inspection results and fitness for service studies.
In addition to his consulting work, Dr. Ivanovic is actively involved in providing technical training courses tailored to industry professionals in Canada, the USA, and internationally. These courses address various levels of complexity, ensuring comprehensive professional development.
